qtmonitor: add Max Connections pane, fix Reset Layout
New pane showing per-IP, per-server max concurrent connection strike
counts from the max_concurrent/{ip} MQTT topics. Color coded: magenta
at 5+ strikes, red at 10+.
Fix Reset Layout to tabify all data panes together (Activity was
previously left with the log panes).
